Transition planning for disabled and autistic adolescents is exactly what it sounds like. Yes momma… yes daddy… you will cut those apron strings and help your child become a productive adult. Your baby is growing up! As scary as that may seem it is actually a great time in your lives. It’s a new adventure for you and your child; an opportunity to start a relationship with a person you nurtured infancy and see the results of your effort. So exciting!

Maryland Department of Education describes transition planning the following way

Transition Planning Guide focuses on the movement of your child with disabilities from school to appropriate postsecondary outcomes and supports the preparation for moving on to the next stage in life. Not every student will have the same goals and outcomes. All students should have goals for future employment whether it is full or part time or employment that includes assistance to be successful. All students should have goals and outcomes for postsecondary education or training that they will need to reach their employment goals. Your child may even have specific independent living goals.

Transition services are further defined as a coordinated set of activities for a student with a disability that: • Is designed to be within a results-oriented process, focused on improving the academic and functional achievement of the child with a disability to facilitate the child’s movement from school to post-school activities, including postsecondary education; vocational education; integrated employment; continuing and adult education; adult services; independent living or community participation; • Is based on the individual child’s needs, taking into account their strengths, preferences, and interests; and • Includes instruction, related services, community experiences, the development of employment and other post-school adult living objectives and when appropriate, acquisition of daily living skills and functional vocational evaluation.

The transition plan is developed with your child’s school. It prepares you, your child, and the school for his transition out of school and into the world. The plan can include college, housing, or entering the workforce, but either way you want to know what resources are available and what type of support your child will need as they become a adult.
